On August 8, 2021, an unauthorized party accessed the Repro Source network. Ransomware was discovered on the morning of August 10. Repro Source immediately launched a comprehensive investigation to determine the cause and scope of the incident.
Repro Source undertook an extensive analysis of their files to determine which individuals and data may have been affected and, on September 24, 2021, Repro Source identified individuals potentially affected.
In an abundance of caution, Repro Source is now notifying all individuals who were identified as potentially having sensitive data obtained during the incident.

Based on the investigation to date, some of your personal information was in files that may have been accessed or acquired without authorization.
This information included your name and one or more of the following: address, phone number, email address, date of birth, billing and health information, such as CPT codes, diagnosis codes, test requisitions and results, test reports and/or medical history information, health insurance or group plan identification names and numbers, and other information provided by you or your treating physician.
