On December 2, 2021, WellDyne became aware of suspicious activity related to the WellDyne company network. WellDyne began an investigation into the activity with the assistance of third-party forensic investigators to determine the nature and scope of the incident. It was determined that there was unauthorized access to the WellDyne network between October 30, 2021, and November 11, 2021.
WellDyne was founded in 1990 and specializes in mail order pharmacy, central fill, clinical expertise, and adherence programs.

The investigation has determined that the incident may have resulted in unauthorized access to some personal information. The information at issue varies by individual and may include:
